COVID WAIVER FORMS

In the coming days, you will receive an email from our inimitable Player Agent Kathy Vigil entitled "Covid Waiver Form- Please Read." You may have already received this email.

DO NOT DELETE THIS EMAIL.

This email includes two forms that you need to digitally sign and return: (1) The Los Angeles Parks and Recreation Consent and Acceptance form, and (2) the North Venice Informed Consent form.

Both of these forms are required for ALL players and volunteers before the city will issue us the necessary permit to run league activities this year. Let us say that again- we need 100% participation on this, or we will not be issued our permit.

You may have already received and signed these forms. If you have done so, we thank you profusely. If you have not yet received it, please expect that email in the next day or so.

If you have received it, and have taken no action, deleted it, or left it sitting in your SPAM folder, please put it right back in your inbox and sign it for us, please.

Please reach out to us if you have any questions on this. Thank you in advance for your assistance in taking this step towards getting the permit we need to get on the field and GET AFTER IT!
